Add Decisions

Explanation

Use this activity to add a new Decision or Action to perform through FMECA Analysis. A Decision determines the action needed to mitigate the functional failures of the chosen Item Class. This process involves selecting the appropriate maintenance actions to address specific Failure Modes. Maintenance actions may include preventive maintenance, corrective actions, or allowing the equipment to run to failure, depending on the nature and severity of the failure mode.

To define a new Decision, start by selecting the appropriate options from the drop-down for Decision which was defined in FMECA Decisions Basic Data page. Then select the appropriate Task Template that suits the maintenance action. The Task Template defines the work list, materials, and resources used to perform the maintenance action to mitigate the failure.

A previously existing Work Task Template or a newly created Work Task Template at the Functional Failure mode level can be selected from the Task Template drop-down. The Task Template Revision field fills in by default when the Task Template field updates.

Note: A Decision can be a simple action chosen without defining other fields. For example, if the action is a documentation update, there is no need to fill in the other fields.

Next, fill in the appropriate Cause defined in the basic data, which determines the cause of the functional failure handled by this Decision or Action.

If the Decision involves performing a maintenance action (Preventive, Corrective, or Run to Failure), select it from the Action Type drop-down.

If Preventive Action Type is selected, the Trigger Type should be visible with the possible values "Calendar", "Condition",and "Event".

Note: When Trigger Type "Condition" is selected, the Performed Value Based field defaults to "No." However, if the Parameter Type is selected as "Accumulated" from the drop-down, the Performed Value Based drop-down will be enabled, allowing the value to be changed to either "Yes" or "No."

If a new Decision is created with the same Trigger Type and Action Type that already exists, an error will be raised: "Same trigger type and action type combination cannot be added as Decision."

If a new Decision is created with the Run to Failure Action Type for the second time, an error will be raised: "Run to Failure Action Type Already Exists."

For example, for a crane with a Failure Mode of reduced trolley speed, choose "Regular lubrication maintenance" as the Decision and "WTT 01" as the Task Template. The Task Template Revision (Rev) field auto-fills when the Task Template field is updated, indicating the current revision of the selected Task Template. Enter the cause of the Failure Mode in the Cause field, for example, "Insufficient Lubrication in Trolley Motor Bearings"

Select the type of action to be taken from the Action Type drop-down if it is a maintenance action. Select the relevant parameters according to the action type.
